Provide the Air Quality Management Facility ID Number.  If you do not know your Air Quality Management Facility ID number, you can look it up on DNREC’s Environmental Navigator at: http://www.dnrec.state.de.us/DNRECeis/.  To look up your Facility ID number, run a search on your facility name.  When the results are displayed click on the “Data View” button for the “Air Emissions” Interest Type.  Your ID number will be displayed at the top of the page next to “Program ID.”

Provide the location coordinates.  Location coordinates are the latitude and longitude of the site or the State Plane Coordinates.  If using the State Plane Coordinates, provide the information in meters.  The latitude and longitude based on North American Datum 1983 (NAD83) can be obtained from United States Geological Survey (USGS) Quadrangle maps.  The State Plane Coordinates, Delaware Coordinate System of 1983, can be obtained from a consultant (surveyor, engineer, etc.) or USGS Quadrangle maps.  You can also contact the Department at (302) 323-4542 or (302) 739-9402 for assistance.  In addition to providing the coordinates, also provide what the coordinates represent (i.e. the front door of the building, the main gate, the center of the facility, etc.) and how the coordinates were generated.

Provide whether the facility is new or existing.  A new facility is any facility that is not currently permitted by the Department or any facility that is currently permitted by the Department that will be modified in such a way that the fixed capital cost of the new components will exceed 50% of the fixed capital cost of a comparable entirely new facility.

Indicate whether the application is for a new source or for a modification to an existing source.  A new source is any source that is new to the location or any source that will be modified in such a way that the fixed capital cost of the new components will exceed 50% of the fixed capital cost of a comparable entirely new source.  An existing source is any source does not meet the definition of a new source and that is currently under construction or operating at the location.

Provide the Responsible Official name.  A Responsible Official means one of the following:

For a corporation: a president, secretary, treasurer, or vice-president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision making functions for the corporation, or a duly authorized representative or such person if the representative is responsible for the overall operation of one or more manufacturing, production, or operating facilities applying for or subject to a permit, and either:

1.                   The facilities employ more than 250 persons or have gross annual sales or expenditures exceed $25 million (in second quarter, 1980 dollars); or

2.                   The delegation of authority to such representative is approved in advance by the Department.

 

For a partnership or sole proprietorship: A general partner or the proprietor, respectively, or the delegation of authority to a representative approved in advance by the Department.

 

For a municipality, state, federal, or other public agency: Either a principal executive officer or ranking elected official.  For purposes of Regulation No. 30, a principal executive officer of a Federal agency includes the chief executive officer having responsibility for the overall operations or a principal geographic unit of the agency (e.g., a Regional Administrator of EPA); or

 

For affected sources:

 

1.                   The designated representative insofar as actions, standards, requirements, or prohibitions under Title IV (Acid Deposition Control) of the Act, or the regulations promulgated thereunder are concerned; and

The designated representative for any other purposes under Regulation No. 30.
