The Air Quality Management Section uses a building block approach to permit applications. This building block approach allows you to custom design your application to meet your specific project needs. There are five categories of forms in the application form series. All Regulation No. 1102 applicants other than auto body shops, dry cleaners, gasoline dispensing facilities, gasoline tank truck (delivery vessel) operators, and air stripping/soil venting operations will use this series. Please Note: All forms should be saved to your computer before you begin filling them out.
All Regulation No. 1102 applications must include one AQM-1: Administrative Information form. This form provides all of the administrative information regarding your application. Instructions for Form AQM-1 are also provided.
All Regulation No. 1102 applications must include an AQM-2: Overall Process Flow Diagram form for EACH process being applied for. This form provides the overall process flow and indicates which forms are being included in the application.

All Regulation No. 1102 applications must include one AQM-3: Process Equipment form for EACH emission unit in EACH process being applied for. These forms provide emission units specific information.
