The use of these forms is not mandatory, but they may be useful to generator owners who may not already be keeping complete and accurate records. Please also view the separate “EXAMPLE” copies of each form which have been filled out with example data, to help generator owners understand how to record the information on the forms.
- Compliance Verification Forms. The submittal of these forms verifies to the Department that an owner of a generator is complying with the requirements of Regulation No. 1144. Please download one of following forms, based upon what type of generator you own (existing vs. new, or emergency vs. distributed):
